A previously well 5-month old girl presented
A previously well 5-month old girl presented with a two day history of fever up to 38˚С and worsening spastic cough. 6 hours ago the child has developed dyspnea and wheezing. The girl was born at term by normal vaginal delivery without any complications, with body weight 3200 g, body length of 50 cm. Artificially fed since the first month of life. Family history is positive for food and drug allergy. At the age of 3 months she was symptomatically treated for URI (low grade fever, serous nasal discharge). Objectively: body weight – 9 kg, length – 65 cm. The girl breathes with severe intercostal and suprasternal retractions and nasal flaring. Respiratory rate is 60/min. Percussion revealed a hyper-resonant lung sound. On ausculatation the girl demonstrates bilateral wheezes and fine crackles. Heart borders are normal. Heart sounds are muffled, heart rate – 140/mines. Fever up to 38,6ºÐ¡. Abdomen is moderately distended, painless on palpation. Liver is 2 cm.
