Case 1 JP is a 72-year-old man
Case 1 JP is a 72-year-old man with a past medical history significant for coronary artery disease, diabetes, hyperlipidemia, and peripheral vascular disease. He is an avid golfer and prefers to walk the course, but this is becoming progressively more difficult for him due to frequent angina and leg pain. He has had two stents placed after a STEMI. A coronary angiogram performed 1 month ago revealed significant disease in the RCA proximal to his graft, but this was considered high risk for intervention. He feels depressed, has not been eating or drinking, and has stopped taking all of his medications except ASA for pain. He thinks that he takes too many medications and does not think they are doing anything. His wife forced him to come in for a visit. Social History: TJ reports social drinking and is a current smoker, smoking approximately one pack per day. TJ lives at home with his wife and is a retired car salesman. Review of the chart reveals: • Allergies: N/A • Vitals: BP 138/86 mmHg, HR 74 bpm, and RR 14 rpm • Ht: 5’9″, Wt: 216 lbs Labs (from today): • A1c of 6.8% • Blood glucose of 128 mg/dL
