Part 1: Rose sustained a burn to her lower left leg when she leaned against the open oven door. The wound was slow to heal and she had required daily dressing changes by the Nurse Practitioner (NP). Finally, after 3 weeks of treatment her wound had improved enough for her to only be seen three times a week. Rose was delighted with the progress and looking forward to only visiting the clinic 3 times a week. Because of the progress, the dressing changes were delegated to the health care assistant (HCA). Care continued with the HCA doing dressing changes for 3 weeks. The 4th week the HCA was on vacation so the NP did the dressing change and noticed an odor and drainage that had not been present 3 weeks earlier. QUESTIONS: What concerns do you have regarding the wound care of this patient? What could have been done to improve delegation of this patients wound care? What do you think has happened? Who is responsible?After seeing Rose, the NP reviewed the notes on the previous visits. The documentation was not complete with no description of the dressing changes or the appearance of the wound.
